About the Role: Health Coach Consultant RN Chronic Condition Coach
We're seeking an experienced Health Coach Consultant RN to work remotely from the comfort of your own home. As a Health Coach Consultant RN, you'll play a vital role in empowering our members to achieve optimal health status by providing personalized coaching, education, and support. This is a full-time, 40 hours/week remote opportunity that's available immediately.
Key Responsibilities:
- Utilize a collaborative process of assessment, planning, implementation, and evaluation to engage, educate, and promote member's decisions related to achieving and maintaining optimal health status.
- Conduct comprehensive evaluations of member needs and benefit plan eligibility for available integrated internal and external programs/services.
- Assess member's level of health literacy, technology capabilities, and/or readiness to change using various assessment techniques.
- Apply and interpret applicable criteria and guidelines, health/wellness management plans, policies, procedures, and regulatory standards to enable appropriate utilization of services.
- Consult with supervisors, Medical Directors, and others to overcome barriers to meeting goals and objectives.
- Identify and escalate quality of care issues through established channels.
- Utilize influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes.
- Provide coaching, information, and support to empower members to make ongoing independent medical and/or healthy lifestyle choices.
- Develop and monitor established plans of care in collaboration with the member and/or attending physician.
- Utilize internal policy and procedure in compliance with regulatory and accreditation guidelines.
Essential Qualifications:
- Hold an unrestricted RN compact license in your state of residence and have the ability to be licensed in all non-compact states.
- 3+ years of recent clinical experience in Adult Medical Surgical.
- 3+ years of recent clinical experience providing health education about chronic health conditions to adults.
- 3+ years of experience with documenting clinical notes in an electronic medical records (EMR/EHR) system.
- 3+ years of proficiency with Microsoft Office applications: Word, Outlook, Excel, Teams, and PowerPoint.
- 1+ year of experience with delivering health education coaching via telephone 1:1 and via webinar-based platform virtually to a group of attendees.
Preferred Qualifications:
- Previous telephonic chronic condition coaching experience.
- Virtual group coaching experience via webcam.
- Managed care experience.
- Experienced in Epic EMR database functionality and documentation.
- Excellent communication skills and comfort with public speaking.
- Ability to work independently as well as part of a group.

Work Schedule:
As a Health Coach Consultant RN, you'll work a standard Monday-to-Friday schedule from 9:00 am - 5:30 pm in your time zone, with 3 set days a week and 2 set evening shifts per week from 12:30-9:00 pm in your time zone. You'll also be required to work rotating late Friday shifts until 9 pm your time zone (typically every 4-6 weeks) and rotating Saturdays (typically every 4-6 weeks).
