Remote RN Care Coordinator (Part-Time to Potential Full-Time) We are seeking a dedicated and experienced Registered Nurse (RN) Care Coordinator for a fully virtual position supporting population health and care coordination efforts. Candidates must hold an active RN license in New York, Texas, or Oregon. This role will begin as part-time with the potential to transition into a full-time opportunity based on performance and program needs. The ideal candidate will have prior experience working in an Accountable Care Organization (ACO) or similar value-based care environment. A background in acute care nursing and proficiency with Salesforce or other care management platforms is highly desirable. Key Responsibilities: ??Provide remote care coordination for a panel of patients in alignment with ACO goals ??Conduct telephonic outreach, health assessments, and care planning ??Collaborate with interdisciplinary teams to ensure appropriate care transitions and follow-up ??Identify and address gaps in care, social determinants of health, and patient barriers ??Document all interactions and interventions in Salesforce or designated system ??Monitor outcomes and escalate complex issues to clinical leadership as needed Requirements: ??Active RN license in New York, Texas, or Oregon (required) ??Minimum 3 years of clinical nursing experience, with a preference for acute care ??Prior experience in ACO, care management, or population health ??Comfortable with technology and remote work platforms ??Strong organizational and communication skills Preferred Qualifications: ??Salesforce or care management platform experience ??Experience with virtual/telehealth care coordination ??Bilingual skills a plus
