We are seeking an experienced Hospice Patient Care Coordinator to join our team in Pocomoke City, MD. As a Patient Care Coordinator, you will provide clinical and management support to the Clinical Nursing Supervisor, ensuring the quality and safe delivery of hospice care services. You will be responsible for the day-to-day clinical operation of the Hospice program, assisting with staffing and scheduling, admissions, and nursing visits as needed.
