Job Summary:
In this role, you will apply your advanced clinical judgment to facilitate comprehensive care planning, collaborate with healthcare providers, and coordinate psychosocial wraparound services. You will work closely with members, their families, and healthcare teams to assess needs, develop strategies, and provide crisis intervention. Your expertise will help us deliver optimal, cost-effective outcomes and improve the overall well-being of our members.
Key Responsibilities:
- Conduct comprehensive assessments of member needs and eligibility
- Develop strategies to reduce risk factors and address complex clinical indicators
- Perform crisis intervention and refer members to appropriate providers
- Collaborate with healthcare professionals to influence member care
- Coach members to make informed healthcare decisions and adopt healthy lifestyle choices
- Document member interactions and care plans in our electronic health record system
- Travel to member locations, including acute mental health facilities, homes, and community settings (up to 25-50% of the time)
Requirements:
- Independent New Jersey License: LCSW, LMFT, LPCC
- Residency in the state of New Jersey
- 3+ years of experience in behavioral health or social services
- 2+ years of experience with personal computers, multi-system navigation, and MS Office Suite applications
- Masters degree required
Preferred Qualifications:
- Bilingual
- Case Management experience
- Experience working on a multi-disciplinary team
