Join our mission-driven healthcare team and leverage your expertise from the comfort of your home. We are seeking compassionate, detail-oriented RNs, LPNs, an MAs to help manage patients with chronic conditions through remote patient monitoring and chronic care management programs.
Key Responsibilities:
• Review remote patient monitoring data (e.g., blood pressure, glucose, weight) and identify trends or concerns
• Conduct monthly check-ins with patients to support care plans and medication adherence
• Coordinate with primary care providers and specialists as needed
• Document patient interactions and care coordination activities in electronic health records (EHR)
• Provide education and encouragement to patients managing chronic conditions
Qualifications:
• Active or recently retired RN, LPN, or MA (current license preferred but not always required depending on state)
• Minimum of 2 years of nursing experience, preferably in primary care, home health, or chronic disease management
• Strong communication skills, with a caring and patient-centered approach
• Comfortable using technology (basic EHR systems, remote monitoring platforms)
• Access to a secure and quiet workspace with reliable internet connection
