Job Summary

As a LPN - Home Health with Enhabit, you'll play a vital role in providing nursing care to patients in their homes under the direction of Registered Nurses (RNs) and in compliance with physicians' orders. Key Responsibilities

• Visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders

• Collaborate with RNs and other healthcare professionals to develop and implement patient care plans
• Maintain accurate and up-to-date patient records
• Communicate effectively with patients, families, and healthcare teams

Qualifications

Essential Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) in the state of employment
• Valid state driver's license
• Automobile liability insurance
• Dependable transportation kept in good working condition
• Ability to drive an automobile in all types of weather conditions
