Job Summary and Qualifications
As a Home Health LPN, you will administer quality, skilled nursing care to patients in their homes, working in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services. You will be responsible for:
- Providing patient care and identifying physical, psychosocial, and environmental needs
- Verifying the Plan of Care prior to each visit and providing care according to physician orders, assessment data, established standards, and guidelines
- Incorporating short and long-term goals into nursing care, as evidenced by documentation in nursing notes
- Performing ongoing assessments during each visit and documenting data in patient medical records
- Communicating significant findings, problems, or changes in patient condition to the supervising RN and/or Director of Professional Services and the physician
- Documenting nursing interventions, including patient response
- Assessing patient/caregiver knowledge of disease process, treatment regimen, and/or medication administration
- Providing basic and specialized teaching as ordered in the Plan of Care and based on patient needs
- Identifying and recommending other disciplines needed for quality care of the patient
Requirements and Qualifications
To be successful in this role, you will need:
- Basic Cardiac Life Support certification (obtained within 30 days of employment start date)
- Licensed Practical/Vocational Nurse (LPN/LVN) licensure
- Driver's license
- Vocational school graduate
- Multistate License or a combination of Missouri and Kansas state license (preferred)
- 1 year of experience in home health care (required)
