As a Field Community Health Worker, you'll play a vital role in managing complex medical or behavioral needs for our members, working collaboratively with our interdisciplinary clinical care management team to advance interventions and promote health equity.
Key Responsibilities:
- Partner with clinical care management team to manage complex members in the community
- Locate and engage members in the community, developing individualized plans of care that focus on the member's definition of success
- Conduct screenings to assess member medical, behavioral, functional, cognitive, and psychosocial needs
- Research, locate, and make referrals to community resources to address member-specific care or socioeconomic needs
- Perform targeted activities and provide education to support HEDIS/STAR gap closure and health promotion

Requirements:
- High School Diploma/GED (or higher)
- 3+ years of experience working within the Community Health setting in a field-based healthcare role
- 2+ years of experience working within the local community, with demonstrated knowledge of culture and values within the community
- Intermediate knowledge of Medicare and Medicaid benefits
Working Schedule and Location:
Schedule is Monday through Friday between 8 am and 5 pm, with a home-based office and local travel up to 25% (mileage reimbursed at the current government rate). Locations will vary within the following counties of Florida: Hardee, Highlands, Hillsborough, Manatee, and Polk.
