Job Summary
We're seeking a highly skilled and compassionate **Registered Nurse Case Manager - Complex Care** to join our team in Washington State. As a remote-based position with occasional field visits, this role offers the flexibility to work from home while still engaging with our community. You'll be responsible for assessing and interpreting customer needs, identifying solutions to complex problems, and providing guidance and support to others. - Assess and Interpret Customer Needs: Evaluate customer requirements and develop comprehensive plans to address their complex needs.

Requirements and Qualifications
Essential Qualifications:
- Active, Unrestricted RN License: Current, unrestricted Registered Nurse license in the state of Washington.
- Microsoft Office Proficiency: Proficiency in Microsoft Word and Outlook.
- Field Visit Requirements: Ability to travel up to 25% within a 60-mile radius from residence to make field visits.
- Residency Requirement: Currently live in Tacoma, Olympia, Seattle, or Everett, WA.
Preferred Qualifications:
- Certified Case Manager (CCM) Certification: Certification as a Case Manager is highly desirable.
- Community Health Experience: 1+ year of experience working in a community health setting or providing community-based care management.
- Complex Care Experience: Experience managing populations with complex medical or behavioral needs.
- Home Care Based Services Experience: Experience working with Home Care Based Services and/or patients in community and home-based settings.
- Case Management Experience: Experience with case management, discharge planning, and related activities.
- Field-Based Work Experience: Experience with field-based work and ability to work independently.
- Familiarity with NCQA Standards: Knowledge of NCQA standards and guidelines.
