Key Responsibilities
- Coordinates and oversees all aspects of patients' home care, ensuring comprehensive and personalized care plans.
- Collaborates with patient care teams to develop and implement care plans tailored to individual patient needs.
- Educates families on patient condition, needs, and care, empowering them to provide support and care at home.
- Performs head-to-toe physical assessments, checks vital signs, and manages medications to ensure patient health and safety.
- Administers injections, IV therapy, or tube feedings as necessary, and maintains catheters with precision and care.
- Completes documentation timely and accurately, ensuring that patient records are up-to-date and compliant with regulations.
- Supervises LPNs and HHAs, providing guidance and support to ensure a cohesive care team.
- Regularly communicates patient progress to the clinical manager, physician, and care team, facilitating collaborative care.
- Other duties as assigned, demonstrating flexibility and a commitment to patient care.
Qualifications and Requirements
To be considered for this rewarding role, you must meet the following qualifications:
- One year of clinical experience as a registered nurse (RN), or less than 1 year with approval from VP Clinical and participation in the RN Intern program.
- Current RN license specific to the state(s) you are assigned to work, ensuring compliance with state regulations.
- Minimum six months of home health experience, providing a foundation in home care practices.
- Current CPR certification, demonstrating a commitment to patient safety.
- Valid driver's license, reliable transportation, and liability insurance, facilitating travel to patient homes.
- Ability to assess patient status and identify requirements relative to age-specific needs, showcasing clinical expertise.
- Familiarity with general use and functions of computers, including user names and password concepts, internet, e-mail, and navigation of computer desktops, to effectively manage online HR services and training programs.
Preferred Qualifications
