Job Overview
The Licensed Practical Nurse (LPN) is a critical component of our home healthcare team, responsible for delivering quality patient care in accordance with developed care plans and physicians' orders. Key Responsibilities:
- Deliver high-quality patient care in compliance with physician orders and under the direction of your Registered Nurse teammate
- Perform hands-on client care, including documentation and timely communication with the client's physician and other caregivers
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent or reduce risk
- Educate clients and their family members based on the client's specific needs
- Provide care in patients' homes using a variety of skills, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes
- Collaborate with our clinical team to develop and implement individualized care plans that meet the unique needs of each patient
- Participate in ongoing education and training to enhance your skills and knowledge, staying up-to-date with the latest developments in home healthcare
Essential Qualifications:
To be considered for this exciting opportunity, you will need to meet the following essential qualifications:
- An active LPN License in the state of Florida
- Valid CPR certification
- Basic understanding of Oasis and Medicare Skilled Nursing experience (preferred)
- At least 1 year of LPN experience in a home health or healthcare setting (preferred)
- Home Care Home Base experience (preferred)
Preferred Qualifications:
While not essential, the following qualifications are highly desirable:
- Previous experience working in home healthcare, preferably with a Medicare-certified agency
- Strong understanding of Oasis and Medicare regulations
- Experience with electronic medical records (EMRs) and other healthcare technology
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
