As a Care Coordinator, you will play a vital role in providing proactive intervention and telephonic coordination of care to eligible members, ensuring they receive appropriate screening, assessment, services, and care transitions. Key Responsibilities: • Conduct telephonic outreach and engagement activities for members eligible for Tailored Care Management and NC Healthy Opportunities Pilot services. • Administer screenings and assessments to identify member needs and develop person-centered care plans. • Link members to appropriate care and services, including locating providers, scheduling appointments, and providing education on health topics. • Manage transitions of care between settings, ensuring seamless discharge planning and follow-up. • Assess eligibility for the NC Healthy Opportunities Pilot and link eligible members to these services using the NCCARE360 software platform. • Document activities in designated software platforms according to Vaya policy and procedure. Essential Qualifications: • Bachelor's degree in a human service-related field (such as Psychology, Social Work, etc.). • Two (2) years of post-degree progressive experience providing similar services to the population served. • Exceptional interpersonal skills, effective oral and written communication skills, and the ability to make prompt independent decisions. • Problem-solving, negotiation, and conflict resolution skills. • Knowledge of Behavioral Health/I/DD Tailored Plan (Tailored Plan) eligibility and services. • Working understanding of whole-person health and health-related resource needs. Preferred Qualifications: • Call Center (inbound/outbound) experience. • Tailored Care Management experience. • Care Coordination experience. • NC Healthy Opportunities Pilot experience. Skills and Competencies: • Diplomacy and discretion to effectively negotiate and resolve issues. • Detail-oriented with ability to organize multiple tasks and priorities. • Proficient in arenaflex Office products (Word, Excel, PowerPoint, Outlook, Teams, etc.). • Ability to work in a fast-paced environment with changing priorities.
