About Us:
LPW Evaluations is a California-based telemedicine platform providing medical evaluations
that support treatment plans for mental health therapists (LMFT, LCSW, LPCC) and physical
therapists. We perform disability evaluations, FMLA/work-leave documentation, work
restrictions, medical necessity letters, orthopedic/functional assessments, and more. No
ongoing treatment is required—these are evaluation-only visits with structured templates.
Position Overview:
We seek California-licensed clinicians (MD/DO/Psychologist/Psychiatrist) to perform
remote telemedicine evaluations. Responsibilities include:
• Work restrictions and return-to-work clearances
• Medical necessity documentation
• Mental health–supportive medical documentation
• Orthopedic/functional limitation assessments
• Disability and FMLA evaluations
• Imaging/lab recommendation assessments

Qualifications:
