Telehealth Physical Therapist (PT)
Primarily supporting Keizer Nursing & Rehabilitation Center (Keizer, OR) and French Prairie Nursing & Rehabilitation Center (Woodburn, OR)
Compensation: $45–$55/hr (DOE)
Schedule: PRN
Work Location: Remote

What You’ll Do

Provide physical therapy services via telehealth to residents in our Oregon facilities

Conduct evaluations and develop individualized treatment plans

Guide residents through exercises, functional training, and pain management strategies virtually

Educate residents and families on safe use of equipment and exercise techniques

Document patient progress accurately and on time

Collaborate remotely with on-site nursing, therapy, and interdisciplinary care teams
What We’re Looking For

Active Physical Therapist license in Oregon (required)

Graduate of a CAPTE Accredited Physical Therapy Education Program

Experience in post-acute care or similar healthcare environment preferred
-
Strong communication and collaboration skills with a focus on patient care

Ability to develop and implement effective treatment plans to meet patient goals

Completes documentation and billing accurately and on time

Problem-solving skills and willingness to meet productivity benchmarks
