Job Overview

We're seeking a skilled and dedicated Inpatient Care Coordinator who is a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) to join our team remotely in Nyack, NY. As a Skilled Inpatient Care Coordinator, you will be the crucial link between patients and healthcare personnel, ensuring smooth transitions of care.
