Job Description:
• Conduct virtual musculoskeletal triage and follow-up visits to assess symptoms, functional limitations, and clinical risk
• Apply strong clinical reasoning to determine appropriate clinical recommendations within a virtual care model
• Deliver evidence-based virtual physical therapy interventions focused on functional improvement and symptom management
• Develop individualized care plans aligned with patient goals, functional needs, and long-term outcomes
• Identify clinical needs requiring care coordination and route tasks to Care Coordinators
• Partner with Advanced Practice Providers (APPs) and other clinicians when medical escalation or specialty involvement is indicated
• Communicate and coordinate with patients, care coordinators, APPs, and external referral sources to support continuity across the episode of care
• Maintain accurate, timely, and compliant clinical documentation in accordance with regulatory, payer, and organizational requirements
• Adhere to established clinical protocols, escalation pathways, and quality standards within a value-based care environment.
Requirements:
• Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program
• Active, unrestricted physical therapy license in the state(s) where care will be delivered
• Minimum of 3 years of clinical experience, with preference for 5 years
• Strong clinical reasoning skills and a patient-first approach to care
• Comfort practicing in a virtual environment, including remote evaluations and video-based patient education
• Preferred APTA specialist certification (e.g., OCS, SCS)
• Residence in PT Compact State
• Experience delivering care in virtual or telehealth-based models
• Experience working in interdisciplinary or integrated care teams alongside medical providers
• Spanish language proficiency.
