Key Responsibilities:
- Serve as primary care manager for assigned members, conducting holistic assessments, and developing personalized care plans to address complex health and social needs.
- Collaborate with our multidisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and transitions.
- Provide education, support, and guidance to patients and their families, empowering them to take an active role in their healthcare journey.
- Utilize advanced clinical judgment and critical thinking skills to identify high-risk patients, develop targeted interventions, and prevent hospitalizations.
- Travel 50-75% within the Fredericksburg, Virginia area, conducting face-to-face assessments, and providing care management services to our members.
Requirements:
- Minimum of 3 years of professional/clinical nursing experience, with a preference for experience in care coordination.
- 2+ years of telephonic and/or field care management experience.
- Active Registered Nurse (RN) license in the state of Virginia.
- Strong organizational, communication, and problem-solving skills, with proficiency in electronic health records (EHR) systems.
- Ability to travel 50-75% within the Fredericksburg, Virginia area.
