Job Overview
We are seeking an experienced RN Case Manager, Complex Care to join our team in Washington State. This is a remote position with field responsibilities, approximately 25% of the time, within Jefferson or Kitsap Counties. If you live in or within commutable driving distance to Bremerton, Port Orchard, Port Townsend, or Silverdale, WA, you will enjoy the flexibility to work remotely while taking on challenging and rewarding responsibilities.
Primary Responsibilities
- Assess and interpret customer needs and requirements, providing personalized care and support to individuals with complex medical or behavioral needs.
- Identify solutions to non-standard requests and problems, using critical thinking and problem-solving skills to navigate complex situations.
- Solve moderately complex problems and conduct moderately complex analyses, using data and evidence to inform decision-making.
- Translate concepts into practice, developing and implementing care plans that address the unique needs of each individual.
- Provide explanations and information to others on difficult issues, serving as a resource and mentor to colleagues and stakeholders.
- Coach, provide feedback, and guide others, supporting the development of a high-performing team.
- Act as a resource for others with less experience, sharing knowledge and expertise to promote growth and development.
Requirements and Qualifications
To be successful in this role, you will need:
Essential Qualifications
- An active, unrestricted Registered Nurse (RN) license in WA State, with a strong foundation in nursing principles and practices.
- Proficiency with Microsoft Word and Outlook, with excellent communication and interpersonal skills.
- Ability to travel up to 25% within a 60-mile radius from residence to make field visits, with a valid driver's license and reliable transportation.
- Residence in Jefferson or Kitsap Counties in WA, with a strong understanding of the local community and healthcare landscape.
Preferred Qualifications
- Certified Case Manager (CCM) certification, with specialized training and expertise in case management and care coordination.
- 1 year of experience working within the community health setting in a healthcare facility, with a strong understanding of community-based care and services.
- 1 year of experience providing community-based care management to members receiving long-term care, personal care services, private duty nursing, or home health, with a focus on complex medical or behavioral needs.
- Experience in managing populations with complex medical or behavioral needs, with a strong understanding of evidence-based practices and care coordination principles.
- Experience working with Home Care Based Services and/or patients in community and home-based settings, with a strong understanding of the unique challenges and opportunities of community-based care.
- Experience with case management, discharge planning, and other care coordination activities, with a strong understanding of the healthcare system and its various components.
- Field-based work experience, with a strong understanding of the importance of community-based care and the role of the RN Case Manager in promoting health and well-being.
- Familiarity with NCQA standards, with a strong commitment to quality and excellence in care coordination and case management.
- Demonstrated competency working with Enrollees and/or families who require intensive case management services, with a strong understanding of the unique needs and challenges of this population.
