Job Summary

Key Responsibilities
- Execute patient intervention pharmacy programs and ensure compliance with clinical pharmacy services
- Drive pharmacy financial performance by implementing pharmacy programs to maximize reimbursements while improving patient health outcomes
- Conduct patient consultations and provide clinical expertise to patients, healthcare professionals, and internal stakeholders
- Review patient profiles and medication histories to ensure safe and effective medication therapy
- Provide education and support to patients, healthcare professionals, and internal stakeholders on medication use and disease management
- Collaborate with healthcare professionals to improve patient outcomes and reduce hospitalizations
- Analyze data and clinical outcomes to identify areas for quality improvement and implement changes
- Maintain current knowledge of pharmacy industry trends, regulations, and guidelines
Essential Qualifications
- Bachelor's degree in Pharmacy or related field
- Current pharmacist licensure as allowed by the relevant state licensing authority
- Experience in patient care pharmacy programs and clinical pharmacy services
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficient in Microsoft Office Suite, including Word, Excel, and PowerPoint
