Job Overview

As an Aftercare Case Manager, you will play a vital role in providing support and services to children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. Your primary focus will be on risk management during the designated aftercare period, which may include crisis intervention, collaboration with public and private agencies, and coordination of community and educational services. You will work closely with families and children to address concerns, provide guidance, and connect them with essential resources to maintain their permanency.
