Pharmacist – Home Infusion
Location: Oklahoma City, OK
Position Summary
We are seeking a licensed Pharmacist to support home infusion services by ensuring safe, effective, and patient-centered pharmaceutical care. This role involves clinical review, compounding oversight, interdisciplinary collaboration, and direct patient and caregiver education to support high-quality outcomes in the home setting.
Key Responsibilities
• Assess patient suitability for home infusion therapy
• Design, monitor, and evaluate individualized pharmacy care plans
• Recommend or adjust medication therapies based on clinical data and provider orders
• Prepare, compound, label, and dispense medications in compliance with USP, DEA, and state regulations
• Conduct nutritional assessments and provide education to patients and caregivers
• Collaborate with nursing and interdisciplinary teams to ensure continuity of care
• Monitor controlled substance inventory and promptly report discrepancies
• Provide accurate drug information to healthcare professionals and patients
• Participate in continuing education and professional competency programs
• Adhere to company policies, quality assurance standards, and all regulatory requirements
Required Qualifications
• Graduate of an accredited School of Pharmacy
• Active Pharmacist license in the State of Oklahoma
• One of the following:
• 1+ year of experience in sterile IV admixture procedures
• Knowledge of non-sterile compounding, immunizations, and Medication Therapy Management (MTM)
• Completion of an ASHP-accredited Pharmacy Residency Program
• Completion of an Advanced Pharmacy Practice Experience (APPE)
• Equivalent experience approved by pharmacy leadership
• Proficiency in Microsoft Word, Excel, and PowerPoint
• Strong customer service focus with a commitment to patient-centered care
Preferred Qualifications
• Experience in home infusion, home care, hospital, or clean room environments
• ASHP-accredited Pharmacy Residency completion
• 1+ year of indirect or supervisory experience
Schedule
• Standard business hours with flexibility to support home infusion coordination
• Rotating pharmacist schedule (approximately every 8th Saturday)
• Weekday shifts: 7:00am–3:00pm, 9:00am–5:00pm, or 11:00am–7:00pm
• Saturday shift: 9:00am–5:00pm
