Responsibilities:

- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Utilize care management tools and resources to support members' needs and goals
Requirements:
- Minimum 2 years of experience in behavioral health or social services
- Willingness to travel up to 50-75% of the time to meet with members in person
- Reliable transportation and ability to reimburse mileage according to company policy
- Active IL BH clinical license (LCSW, LCPC, LMFT)
Preferred Qualifications:
- Case management and discharge planning experience
- Managed care experience
- Microsoft Office experience
