Hera provides a dedicated care manager in the community, covered by Medicare, to be the trusted guide to age at home. We help the family just be family again.
We’re looking for mission-driven social workers or nurses, ideally with experience in hospice, palliative care, or geriatrics, who are passionate about making a difference in the lives of older adults and their families. This is a flexible, remote, 1099 opportunity with the path to grow into a leadership role on our team.

You’d Be a Great Fit If You:
• Love creating connection with families and older adults
• Find yourself to be a creative problem-solver and want a job that looks different every day
• Are tech-savvy and excited to work in a fast-paced, early-stage environment
• Hold an LMSW, LCSW, or RN/BSN (clinical license is a plus but not required)
• Have 2+ years of experience in care management, case management, hospice, palliative care, or geriatrics
• Live in New York or New Jersey (not required but ideal)
