Position Overview We're seeking a skilled Population Health Administrator to join our dynamic team. This role is critical in developing, implementing, and managing our organization-wide strategy for maximizing quality and financial achievement in Population Health and Value-Based Care initiatives. Key Responsibilities ? Lead high-impact, organization-wide population health initiatives ? Convene and lead cross-functional teams on broad-scope projects using advanced performance improvement models ? Develop and execute comprehensive project plans, managing multiple projects simultaneously ? Create annual plans and proposals to meet program goals and targets ? Identify and address potential threats to value-based program performance ? Collaborate with clinical and operational leaders to develop systems for monitoring key performance indicators ? Analyze complex data, interpret findings, and develop actionable reports and presentations ? Serve as a subject matter expert in population health, providing guidance on strategies and policies Qualifications ? Current CA RN license ? Bachelor's degree required; Master's degree in a related field preferred ? 5+ years of related experience in healthcare, particularly in quality improvement and population health ? Strong knowledge of MS Office Suite, especially PowerPoint, Excel, and Visio ? In-depth understanding of Performance Improvement standards, Advanced Payment Models, and Value-Based Care models ? Excellent analytical, critical thinking, and problem-solving skills ? Outstanding verbal and written communication skills ? Proven ability to lead effective teams and manage multiple projects ? Detail-oriented with the ability to see the big picture ? Certification(s) in a related field Preferred Skills ? Experience with EPIC electronic health record system ?
