SeniorsPlus is seeking full-time Home Visitors to assist clients in making informed choices to live safely and independently in their communities. One position is available in each of the following Regions: Northern, Central, and Southern Maine. What You?ll Do as a Home Visitor: ? Perform home visits to clients and ensure timely, accurate documentation is communicated to Care Coordinators. ? Assess clients? health, welfare, and unmet needs while developing backup plans and goals to support their independence. ? Collaborate with Care Coordinators to ensure effective, efficient, and coordinated services. ? Evaluate the client?s plan of care to ensure continuity and effectiveness. ? Positively impact the well-being of disabled children, adults, and older adults participating in at-home, long-term care programs. ? Must have a valid driver?s license and reliable transportation. Schedule: ? Full-time, 8-hour shifts ? Day shift with occasional weekend availability ? Travel is required (mileage reimbursement is provided).
